Fees

As all situations are unique, the fee for a comprehensive neuropsychological assessment depends upon your needs. During the initial telephone interview, Dr. Horsfall will speak with you about your (or if referred for a pediatric assessment, your child's) current difficulties, create a plan for an assessment that is most appropriate for you (or your child), and then discuss with you the cost of the evaluation. The fee covers the interviews (parent, child, referring professional, etc.), background review, all testing, individual and/or family feedback, telephone feedback to the referring professional, and the written report with impressions, recommendations and referrals. Additional letters or meetings, such as IEP attendance or off-site feedback sessions, are not included in the fee. We ask that you please plan to pay for the assessment at the beginning of testing. You may pay with check or cash, and you will be provided with a receipt upon request.

Because reimbursement rules can change at any time we require payment at the time of service, but we will help with the insurance reimbursement process following the assessment. Upon request, we will provide you with a statement describing the services (with the CPT codes required by the insurance company) and diagnostic impressions, which can be submitted to your insurance company for reimbursement.

If you wish to obtain reimbursement, it will be important to check with your insurance company first to determine your coverage. Because neuropsychological assessment is more often considered a medical (not mental health) benefit, insurance carriers may provide reimbursement to patients with a medical diagnosis if the assessment is considered medically necessary. This means that a physician needs the information to establish a diagnosis as the basis for medical treatment, to evaluate the effects of a medical treatment on functioning, or to determine the best course of treatment. Typically, a letter of medical necessity or “prescription” from a physician is required by the insurance company to help facilitate the reimbursement process. Along with this letter, you should submit a request for a “neuropsychological evaluation” with CPT codes to your insurance company for pre-authorization of services.

Please be aware that Eastside Neuropsychology, PLLC does not accept direct payment from insurance companies and is not a preferred provider for insurance companies.
